All fees and payment policies will be discussed before the evaluation or treatment begins.
Payment is due at the time services are rendered.
Visits may be paid for by check or cash.
Dr. Lantz does not accept credit cards.
Insurance
Dr. Lantz will provide you with an itemized bill suitable for presentation to your insurance carrier but are not in any way party to any arrangement you may have with your insurance carrier.
